Change to Your Long-Term Condition Check-Ups
If you have a condition such as asthma, diabetes, or high blood pressure, we check on this every year to help keep you well. We are updating how these yearly checks work, to make things clearer and easier for you.
What will happen now
Instead of one appointment, your check will now usually be split into two separate appointments:
- A check-up appointment: You will see a GP Assistant. This may include simple tests, such as blood tests, blood pressure, weight, or other checks, depending on your condition.
- A review appointment: You will then see a Nurse Specialist or GP. They will go through your results with you, talk about how you are doing, and agree next steps together.
What this means for you
- You will be notified around your birth month that your initial assessment appointment and review are due.
- You will then be asked to book in for your first appointment, and you will be contacted for the second appointment shortly afterwards.
- All your reviews are still happening in the same way as before — only now we have made the process more streamlined and organised.
- We are aware some patients have special circumstances, and we have adjusted the recall system accordingly.
